COACHING AGREEMENT

The client acknowledge and agree that they have been fully informed regarding the nature of services that will be provided by the Health and Wellbeing Coach (Provider). In particular, while the provider believes that the client will benefit from the program, provider has made no representations, promises or warranties regarding the results.

 

CLIENT RESPONSIBILITY

The client acknowledge and agree that adherence and followthrough outside of the coaching session is necessary to succeed. The client agree to collaborate with the provider to set goals , identify priorities, locate support for themselves when needed, etc. The client agree to contact their own doctor for any questions about their ability to exercise or participate in the programs safely. This program is for recreational use only and nothing stated, given or written should be taken as a medical advice.

ASSESSMENT

As a precondition to receiving coaching services, provider may require questionnaires and forms to be completed, as well as a Functional Behavior Assessment (FBA). The FBA conducted by the health coach who is also a Board Certified Behavior Analyst (BCBA), identifies what behavior challenges are and what steps will be taken to reduce unproductive behavior and replace them with productive behaviors. This includes asking questions not limited to the client's diet, relations, lifestyle and preferences.

CONFIDENTIALITY

The information you provide will be kept strictly confidential, except as may be required by law. Data will be stored securely and will be made available only to the client, unless otherwise specified. Behavior Motivation, LLC upholds state and federal law to maintain the privacy of your protected health information (PHI). PHI includes any identifiable information about your physical and mental health, any healthcare you receive and the payment for your healthcare.

FINANCIAL AGREEMENT

All individual sessions are prepaid only. The client agree to pay the full amount of services before starting a program. All payments are final, non-transferable and non-refundable.

CANCELLATION POLICY

The client and provider will agree to set a meeting timeline at the start of the program. The schedule may be modified at any time with advanced notice. Please cancel or reschedule at least 24 hours before the session. Missed sessions, for any reason, will not be reimbursed. In case the provider misses a session, a make-up session will be scheduled. Arrival to a session/call after 15 minutes of your scheduled time will be considered "no show" and the session will count as being used. Regardless of starting time, all sessions will end at the predetermined end time.

TERMINATION OF AGREEMENT

It is within the client's rights to terminate services at anytime for any reason. However, payment is non-refundable. If the provider decides to terminate services for any reason, the provider will return prepayment for unrendered services only.

RELEASE OF LIABILITY

The client voluntarily and knowingly agree on behalf of themselves, spouse, heirs, personal representative, assigns and anyone else claiming by or through the client to WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for death, disability, personal and emotional injury, property damage, theft, or actions of any kind which may hereafter occur to me including traveling to and from this activity, Behavior Motivation, LLC and/or their directors, officers, employees, volunteers, representatives, and agents, and the activity holders, sponsors, and volunteers.
